Maxillary prognathism is a dental and facial condition in which the upper jaw extends forward more than normal in relation to the lower jaw. This condition can affect the overall balance of the face and may lead to difficulties with proper bite alignment, chewing, and speech. In many cases, maxillary prognathism develops due to genetic factors, abnormal jaw growth, or developmental conditions that affect the structure of the facial bones.
Patients with maxillary prognathism often experience an excessive overlap of the upper teeth over the lower teeth, commonly referred to as an overbite. This misalignment can sometimes lead to discomfort in the jaw, uneven wear of teeth, and difficulties in maintaining proper oral hygiene. Early diagnosis is important in order to determine the most suitable treatment approach and prevent further complications. Treatment for maxillary prognathism depends on the severity of the condition and the age of the patient. In mild cases, orthodontic treatments such as braces or aligners may help correct the alignment of the teeth. However, in more severe cases where the jaw structure is significantly affected, orthognathic surgery may be recommended. This surgical procedure repositions the upper jaw to achieve better facial balance and proper bite alignment. II. When do you need a maxillary cosmetic/orthognathic surgery?
The need for surgery generally falls into two categories:
a. Function Related
Malaligned (misaligned) jaws can cause:
- Difficulty in chewing and speaking.
- TMJ problems (jaw joint pain).
- Disrupted sleep and breathing issues, such as sleep apnoea and snoring.

b. Cosmetic Concerns
Various skeletal deformities can affect overall aesthetic appearance, including:
- Gummy smile (Vertical maxillary excess).
- Anterior-posterior discrepancies of the maxilla.
- Constricted arch.
III. Types of Maxillary Jaw Surgery
Le Fort Osteotomy Procedures
- Le Fort I Osteotomy: A procedure that separates the upper jaw from the rest of the facial bones, which facilitates repositioning it forward, backward, superiorly (upward), or inferiorly (downward), thereby creating a better facial appearance.
- Le Fort II Osteotomy: Mainly performed on the midface and cheekbones for a "sunken" or depressed skeletal complex in the midface area.
- Le Fort I Augmentation: A procedure that involves the forward movement of the maxilla (upper jaw), thereby repositioning it to correct a depressed upper jaw problem.
Pre-Surgical Preparation
The notes outline several steps required before undergoing these surgeries:
- Pre-surgical Orthodontics: Often needed before the actual surgery takes place. Imaging: X-rays and CT scans are required.
- Planning: Virtual surgical planning is performed on a computer, along with model surgery.

- Hospital Stay: Typically lasts approximately 3-4 days.
